We invite you – as someone who has used QLife – to participate in our Service User Survey. It's an opportunity to have your voice heard and to contribute to improving the services that we offer to you and other LGBTIQA+SB people and their friends & family.

We want your feedback about your experiences with QLife and your ideas for the future. We would appreciate your honest and constructive feedback to help us deliver the service you need.

This survey should take approximately 15 minutes to complete. Complete the full survey and opt in for a chance to win a $50 voucher.

You can complete this survey without sharing any identifying information.

The survey consists of two parts:

• Part One – 23 Questions about our users and their use of the QLife service.
• Part Two – 15 Questions about the QLife website and how it might be enhanced.

If you choose to complete Part Two, you can choose to remain anonymous or enter a draw to win one of three $50 gift cards. To enter the draw, you will need to provide us with some contact details so that we can get in touch if you are drawn as a winner.

Providing your contact details is completely optional and will only be used to contact you about the gift card draw. Survey results will be reported in a way that does not identify any person who takes part in the survey.
